Aldactone Drug Class and Mechanism

Spironolactone inhibits the effect of aldosterone by competing for intracellular aldosterone receptor in the distal tubule cells (it actually works on Aldosterone receptors in the collecting duct). This increases the secretion of water and sodium, while decreasing the excretion of potassium. Spironolactone has a fairly slow onset of action, taking several days to develop and similarly the effect diminishes slowly. Spironolactone has anti-androgen activity by binding to the androgen receptor and preventing it interacting with dihydrotestosterone

This medicine is a potassium-sparing diuretic used to treat congestive heart failure or high blood pressure. It may also be used to treat other conditions as determined by your doctor.Aldactone is administered at a daily dosage of 400 mg for three to four weeks. Correction of hypokalemia and of hypertension provides presumptive evidence for the diagnosis of primary hyperaldosteronism.

Aldactone Side Effects

Side effects, that may go away during treatment, include abdominal cramping, nausea, diarrhea, headache, or change in sexual ability. If they continue or are bothersome, check with your doctor. Check with your doctor as soon as possible if you experience dry mouth, excessive thirst, unusual tiredness, unusual muscle weakness, skin rash, deepening of voice, irregular or no menstrual periods, slowed heart rate, or enlarged breasts in men or breast tenderness in women. Acute overdosage of spironolactone may be manifested by drowsiness, mental confusion, maculopapular or erythematous rash, nausea, vomiting, dizziness, or diarrhea. Rarely, instances of hyponatremia, hyperkalemia, or hepatic coma may occur in patients with severe liver disease, but these are unlikely due to acute overdosage. Hyperkalemia may occur, especially in patients with impaired renal function.
